Deadline for Teamsters Enrollment

Don’t forget your action is required by December 15^th to maintain your medical coverage and qualify for the 2022 Health Assessment Incentive.

You must complete this in order for your medical claims to be processed in 2022 and this must be done even if you do not want to change any of your information.

You have the option to take your Health Assessment by December 15^th to save $200 per person or $600 per family off your annual deductible.

*Note:  If you are married or have a covered domestic partner, both of you must take the HA in order to get the lower deductibles.
